Person-centered values provide a foundation on which we can base and build our practice. We need to understand what the values are, how we can promote them and why they are important. A value is simply what is important in the life of the person we are supporting. These are the person centered values; Individuality, rights, choices, privacy, independence, dignity, respect, partnership, equal opportunities. 1.2 person centered values are important with care plans as Care planning is all about improving the lives of those who receive care.
Explain the importance of continually improving knowledge and practice. The importance of continually improving my knowledge, is to ensure that the I am aware of, and follow, the current health & social care standards, legislation, and guidelines for good practice. Learning new skills and refreshing training enables me to progress, and achieve within my career and working practices. Personal development not only benefits an individual, but also the organization, and the residents who receive the services. The focus of training tends to be on the staff group delivering the services, to ensure that all recommendations and standards are achieved.
FACILITATE PERSON CENTRED ASSESSMENT, PLANNING, IMPLEMENTATION AND REVIEW UNIT; HSC 3020 1.1 It is important to take a holistic approach to assessment and planning of someone’s’ care and/or support. Holistic means to take into account every aspect of a person so for example not just their physical needs but how these needs affect them emotionally. This is exceptionally important as by looking at every aspect of a person’s needs I am able to provide them with the very best support and care planning that I can offer. 1.2 Whilst conducting a support plan with a tenant I support them to take the lead when planning any support needed. I do this by encouraging them to speak freely and asking open ended questions and leading questions such as “and how would you do this/that without support” and “how do you think you could manage?” I would ensure that the tenant was comfortable in their surroundings which in turn would put them at ease and enable them to talk more freely about any issues that they have enabling me to provide them with the best care and support.
Analyse how systems and processes can promote equality and inclusion or reinforce discrimination and exclusion. When promoting equality, diversity and inclusion in my work place, I will ensure that my behavior, words and actions and those of people working with me support a commitment to equality, diversity and inclusion policy. I make myself aware of personal responsibilities and liabilities under equality legislation. I would also identify the diversity and needs of my staff and service users and identify areas where needs could be better satisfied and where the diversity of the service users could be improved. This is an ongoing process with health care due the changes in needs of individuals who use our service.
Continually improving and adapting approaches benefits both service users and practitioners, ensuring that each individual needs are catered for. This will also enable you to establish what training or mentoring you need. How can standards be used to help a social care worker reflect on their practice? Working in the Health and Social Care industry I think the most important thing is the service users and our health and social care workers and their happiness. The more I reflect and enhance my own knowledge the more effective the learning and training will be. We also have to take responsibility for our own behaviour on people that we manage and train and encourage them to show positive behaviour too. 1.2 – Analyse potential barriers to professional development There are a number of barriers within my role.
