It is vital that this company can show up to date documents, in the forms of policies and procedures, so that employees can gain a better insight into how the company works with others to promote best practice. Unit 517 Lead Person Centred Practise (HSCM1) 1.5 Explain how person centred practise can result in positive changes for an individual. Person centred practise (PCP) places the individual at the centre of the care they receive and this can lead to a lot of positive changes. Before the use of PCP, treatment of an individual was based on their condition (medical model). Decisions are continually made for the individual as they are only defined as being independent once they overcome their disabilities. Taking away a person’s choices long-term may lead to ill effects such as withdrawal, depression, lack of motivation, lack of progression, delusions of over dependency, challenging behaviours, lack of opportunities and community access and institutionalisation.
Also I would think it would be very good if a carer would take some advise from a senior carers and talk to them to gain experience and best advice that you can get from a people who are doing a job longer and know what to expect and know the personnel and clients. If my client is going to share some confidential information with me and I am going to be concern that those action are going to affect his health or others I am going to let him know in a first place that I do have to reported and share those information’s with a manager or senior colleagues to prevent any harm which could be cause by not informing

It also ensures any health and safety issues are recognised and reported. All carers attend hand over at the beginning of each shift and also complete communication books after attending an individual, thereby keeping other staff informed and aware of current situations within the workplace. Individuals communicate with carers to express their needs and preferences and to ensure they are met. As a carer I would discuss the options and choices available to the individual to allow them an informed choice regard their care. Reflective practice means thinking about and evaluating what we do and discussing any changed which could be made means focusing on how we interact with colleagues , customers and the environment. It is also something we think of doing differently which can have positive impact on the care we give customers. It is also means reflecting our own values, beliefs and experiences which shape our thoughts and ideas. Patients quite rightly expect to be well cared for by the NHS across all healthcare settings. Care should be high quality, focus on the patient and be safe.
